Financial Assistance Scholarships: Due to the amazing donations that we have obtained, we are happy to announce that wecan fund five (5) Financial Assistance Scholarships. Applications are now being accepted. The Scholarship will assist a family wanting to attend the conference but cannot financially afford it. The Scholarship will pay for the registration fee for up to 4 individuals, the family fun event and hotel stay for three nights. Deadline to apply is April 25, 2025. $200.00 Family Registration is for four individuals of the same family - children or adults. Late registration will be $300.00 after July 1, 2025.
$100.00 Early Individual Registration for one member. Late registration will be $150.00 after July 1, 2025.
$300.00 Family Registration for non 5p- Society members for up to four individuals in the same family. Late registration will be $400.00 after July 1, 2025. Become a member and save $100.00 (membership fee is $25.00/year)
$150.00 Individual registration for one person who is not a member of the 5p- Society. Late registration will be $200.00 after July 1, 2025. Become a member and save $50.00 (membership fee is $25.00/year)
Fill out your registration information below.
Family registrations are for up to four individuals in the same family. Registrants can be either adults or children. After you register the first individual you will be see a box that says Add Additional Registrants, click on the box. Click the "Family Registration - Member or Non-Member" button and it will turn from $200.00 to $0.00 for the remaining three individuals in your group. Keep adding registrants until you have added everyone in your party before checking out. You will have the opportunity to add more than four individuals. Each additional individual will be an additional $50.00 added to your registration.
Individual registrants will only be able to register for one person. You will not be able to add any additional individuals onto your registration.
The Family Outing will be at the National Ability Center (NAC) in Park City, Utah. To help offset the costs we are charging $3.00 per person to attend. There is no charge for your child or adult with 5p- syndrome. We have four questions for you regarding the event. (1) Are you planning to attend the Family Fun Event; (2) How many people in your party will be attending; (3) Will you be transporting yourself to the NAC or will you be taking our transportation (bus); (4) If taking the bus will you need wheelchair accommodations. T-shirts: T-shirts will be included in your conference registration. To guarantee your t-shirt order, please register no later than June 1, 2025
